Description | IP was cleaning the large drum of the Klieverik Heat Transfer Machine 2 and his hand entered the small gap between the roller and belt feed which pulled his hand through the machine. IP has sustained the full loss of four fingers on his right hand, bruising and crush injuries to his thumb, He will never be able to use his right hand again. The company have failed to demonstrate that they had a suitable and sufficient assessment of the risk.There was also a failure to deliver adequate instruction and training. Significantly, there was no safe system of work in place prior to the incident - formally recorded or otherwise. | ||
